- Root Cause of Issue
- Outcomes
- Corrective Action / Preventive Action to be Taken
- Effect on Set Project Constraints (Priority Weightage)
- Reference to related Records / Correspondences
- Claim Amount with Break Up Details (If any)
- Acknowledgement from the other Party with Remarks
